Applications for Fall
admissions, including the statement of purpose and recommendation letters, must
be submitted on-line. Do not download
or print the supplemental application form pdfs.
The IEOR Department accepts only online letters of recommendation.
Submission instructions are below. Additionally, all admissions data should be
submitted online. Only original transcripts should be sent by mail. We require one official transcript from each
college or university you have attended.
Admission:
Prospective
students applying for graduate study
in the Department of Industrial Engineering and Operations Research must first
complete and electronically submit a University
of California at Berkeley
Graduate Student Application: http://www.grad.berkeley.edu/prospective/.
Please note:
1) The application is only considered
submitted when you press the submit bottom.
2) The University of California
will supply you with a unique ID number when you complete and submit your
on-line application. Please write this ID number down.
Readmission: If you have ever been registered as a graduate
student at Berkeley please complete a readmission form located at registrar.berkeley.edu/elecforms/ReadmGrad.pdf
and mail to the Student Service Office University of California, Berkeley,
Department of Industrial Engineering and Operations Research 4141 Etcheverry
Hall, Berkeley, CA 94720-1777.
Reactivation: If you submitted an application within the past
year, but did not register, you only need to complete and submit a new Form A,
and pay a $60 fee to reactivate your application.
- Indicate on the form the most recent semester that you applied.
- You must meet all the same requirements and deadlines as first time
applicants
- You must also submit updated transcripts showing any work that has
been completed since your previous application.
Letters
Of Recommendation:
All letters of
recommendations are to be submitted online.
Letters
of Recommendation cannot be submitted prior to the submission of your
application – There is no exception to this rule.
- After you
submit your application to the Graduate Division you will receive an
acknowledgement via email from the IEOR department.
- The email
will include an 8-character password and a website to visit, where you
will complete your application process.
- It may
take some time (1-2 weeks) for the University to transfer the application
to the IEOR department, and for you to receive the confirmation e-mail and
password, please be patient.
- The email
acknowledgement will contain instructions for you to complete your waivers
of access to the letters of recommendation online. THIS STEP IS REQUIRED.
- Once the
electronic waivers are completed, you will be asked to enter the e-mail
addresses of your recommenders (at least 3 recommenders).
- Your
recommenders will in turn get e-mail instructions on how to submit their
letters online. You may want to notify your recommenders in advance of
this new online process.
- We will
notify you by email as we receive letters from your recommenders. Please
do not email us for a status of your application before Dec 18th.
Your recommendation letters should be received no later than January 19,
of the corresponding year. ONLY letters of recommendation will be accepted
past the Jan 5th deadline.
